The App enables a Contact Us form to be added to an Office 365 public website. Website visitors can then answer the form by filling in contact information. The contact information is saved in a SharePoint list that is accessible by the website admin/owner. The form can be modified by the website admin to contain one or more pre-defined fields, such as e-mail, Name, Address, etc.
